HOME DELIVERED MEALS


SERVICE AREA:

COSTA MESA, CYPRESS, GARDEN GROVE, LA PALMA, LOS ALAMITOS, MIDWAY CITY, ROSSMOOR, SEAL BEACH, STANTON, AND WESTMINSTER.


WHO CAN RECEIVE MEALS?

Any person age 60 or older, who is homebound, unable to prepare their own meals, and does not have anyone to assist him/her.


PURPOSE OF THE HOME DELIVERED MEALS PROGRAM

To help restore the temporarily ill, permanently homebound, or persons recently released from the hospital, to a healthy state.

To supply participants with nourishing well-balanced meals, who are otherwise unable to obtain them.

To help participants maintain an independent lifestyle and avoid institutionalization for as long as possible.

To provide a friendly visit and social contact by the volunteers who deliver the meals.


HOME DELIVERED MEALS PROCEDURE

A Case Manager makes a home visit to interested individuals to evaluate eligibility for the program.

If the individual is eligible for the program, their meals are delivered as soon as possible.

The hot meals are placed in an insulated food container, which maintains the required temperature for up to two hours.

Cold food items are placed in a cooler with ice to maintain the appropriate temperatures.

Volunteers deliver meals 5 days per week (Monday-Friday), using their own vehicles.

Meals are delivered daily between the hours of 11:30 a.m. and 1:00 p.m.


THE HOME DELIVERED MEALS

There are two meals delivered, a hot lunch and a cold dinner.  Breakfast is available, if needed.

Frozen meals are available for the weekend, if needed, and are delivered along with the regular meals on Friday.

The menu is planned by a Registered Dietitian and varied. Two types of diets are available, a regular diet and a low sugar diet.  All meals are low salt.

Each meal supplies 1/3 of the Recommended Daily Allowance of nutrition and is packaged in disposable containers.

SHARED HOUSING PROGRAM

WHAT IS SHARED HOUSING ?

Shared Housing brings together a person that has a home to share and a person that needs a place to live. Our program not only brings seniors together, but non-seniors as well. The following cities and communities in Orange County are served by Senior Meals and Services' Shared Housing Program:

THE NEED FOR SHARED HOUSING

Housing costs and rental fees are high in Orange County. Seniors seeking Senior Housing in Orange County are experiencing a two to three year wait to obtain an apartment. 87% of seniors over 65 years of age retire on $10,000 per year or less.

IMPLEMENTATION OF THE SHARED HOUSING PROGRAM

The program is announced through press releases, flyers and public service announcements.

Our Shared Housing Coordinator will register home seekers and home providers at their homes and at local senior centers. People are matched through monthly mixers and the Senior Meals and Services Shared Housing Coordinator.

Assistance will be provided in developing agreements between the two parties, reference checking and encouraging discussion of possible problem areas.  /Follow-up assistance is available after the matches have been made.

COMPLEMENTING OTHER SENIOR MEALS AND SERVICES PROGRAMS

Shared Housing has complemented and strengthened our current programs. Through Case Management, In-Home Services, Home Delivered Meals and Congregate Dining Centers, we have identified a strong need for more affordable housing. People seeking housing very often have other needs as well.  Our Case Management Program can assist with those needs.

CASE MANAGEMENT


CASE MANAGEMENT / IN-HOME SERIVCES PROGRAM

WHAT IS CASE MANAGEMENT?

Case Management is a program which links seniors with community service agencies, i.e.: domestic/personal care, medical services, and financial assistance. Services can be arranged on a short-term or long-term basis. The following cities and communities in Orange County are served by Senior Meals and Services Case Management / In-Home Services Program:

WHAT ARE IN-HOME SERIVES?

The In-Home Services Program can provide the client with personal care and homemaking services through vendors contracted with Senior Meals and Services or from our In-Home Services Registry of care providers.

WHO DOES CASE MANAGEMENT / IN-HOME SERVICES SERVE?

The program is available to any person age 60 or older who:

· Is unable to carry on normal day to day activities.
· Does not have anyone to assist him/her.
· Is in need of a range of services to maintain an independent lifestyle.

The Case Management/In-Home Services Programs are not limited to Senior Meals and Services Nutrition Program participants.

PURPOSE OF THE PROGRAM

Through Case Management and In-Home Services, efforts are made to help participants maintain an independent, safe living arrangement and avoid premature institutionalization. Our staff strives to maintain and enhance personal strengths and, whenever possible, encourage natural support systems.
